This form is provided to the Staff Crew of STS Young Endeavour. The information you include will enable your Watch Leader to ensure you receive positive outcomes during your voyage. This is the time to let us know more about you so we can ensure you have the adventure of a lifetime! Please include a recent profile photo for our staff to put a face to your name.

2. Re-upload the signed form
This form is your acknowledgement and acceptance of the Young Endeavour Youth Scheme Terms and Conditions for your voyage. If you are under 18 years of age your parents or guardian will also have to sign. Soon after you return this form to us we will send you an invoice so you can arrange for payment of your voyage fee.

This form and the letter attached to it need to be read and completed by you and a doctor of your choice between 4-6 weeks prior to departure. It provides us with the necessary information to ensure we can safely take you to sea.

If you submit Form C clearly, we will also ask you to complete Form C2. This form should be completed between 4 and 6 weeks before the voyage. It provides us a check in case your health has changed since your Form C submission.
